Consultation letter to patients, breast augmentation
Breast implants were invented in the early 1960s by Dr. Frank Gerow and Thomas Cronin, two Plastic Surgeons in Houston, Texas. The first implants were made from a rubber cover silicone and silicone gel content. They had a remarkable natural feel that a close relative to normal breast tissue youth. The main complication of implants silicone breast was scarring around the implant which was very difficult to obtain and, in some cases very painful.
This is not surprising since all foreign objects are placed in the body cause scar tissue formation. However in the case of stainless steel plates for fractures or a pacemaker, nobody cares if they are difficult because they started that way. In the case of breast implants, she had instead of 80 percent of the time which was a very big deal. After some research, plastic surgeons began to place the implants under the pectoral muscle in an attempt to prevent this "capsular contracture" to form. This has been quite successful and brought the contracture rate to about 30 percent. Better but not perfect.
Then, research was done changing the surface of the implant so-called texture rough. The idea was that the obligations of the irregular surface would be with the tissues of the body better and break down scar tissue and, consequently, prevent contracture. This was much better with contracture rate of nearly 5 percent. However, like everything in medicine, nothing is zero for percent and contracture are still about 5 percent.
So in the late 1980s, there were silicone breast implants with textured surfaces and the results were excellent. In fact, the results were so good that many times it was impossible to tell by physical examination that patients had implants.
In the early 1990s a group of women who underwent breast augmentation and reconstruction began arguing that their silicone breast implants were responsible for disease in their bodies. They were serious, progressive and non-curable diseases such as autoimmune rheumatoid arthritis, lupus and chronic fatigue syndrome. There was no scientific evidence that this was happening, but the claims caused the Food and Drug Administration (FDA) to ban their use.
After several years of litigation and scientific study, it was shown that silicone breast implants have no association with or cause disease in patients. This does not mean they are perfect in every way. The problem Silicone gel is that it is not absorbable by the body. So if it breaks and becomes gel outside the scar tissue which normally covers the implant (which is very rare), there may be some very serious inflammatory tissue that can form and may require intervention surgery to correct.
But this is not a disease. You should always keep in mind that nothing in medicine or surgery is perfect. For example, if you vaccinate your children, as many as one of two hundred thousand may have a severe reaction and even die.
However, at the time of this writing, the implants that are approved for general use are filled with saline. They are made of a silicone rubber shell similar and are inflated at the time of surgery to their appropriate size. The good thing is that if they break, the body absorbs only water with few side effects other than the volume is lost.
However, as you will see during your consultation, saline-filled implants do not feel any something like silicone gel or normal breast tissue. That is why we now place these implants generally under the pectoral muscle. This is a carving technique to hide the implant under normal soft tissue. In this way, the implant is less visible and a more natural feel. They are not yet in As a natural feeling silicone gel, but they are almost always acceptable in appearance and feel for most patients.
There are three options for placement the incision and there are advantages and disadvantages to each.
Implementation of the incision in the armpit is nice because it no incision on the chest. However, the most critical part of breast augmentation is the placement of the implant in the crease under the breast. Since this incision is the farthest from the fold, it is technically more difficult to precisely place the implant. In addition, you should consider this Surgery for maintenance.
For many reasons, it may be necessary for you to undergo additional surgery to improve or maintain the result. Although I have not seen these rates of reoperation in my practice, in recent data from re-operation rate can approach 20 percent in ten years. We never cut through ducts that connect the nipple into the breast tissue and, therefore, all patients should not have difficulty breastfeeding.
However, can you get a nipple numbness of the transaction? The answer is yes. But this has nothing to do with the incision. The nerves that go to the nipple are located near the implant pocket and commonly stretched and bruised. If they are stretched or bruised enough, they may stop working. In the 15 to 20 percent of cases that have numbness after surgery, most will resolve and be normal at 12 months. However, they could be permanently numb. But this has nothing to do with the incision. It is, again, because of the pocket.
There are three reasons why most surgeons perform this incision. On the one hand, it is technically easier to create the precision pocket. Two, if you need another operation you can usually do so through the same incision. However, the main reason why most surgeons use this incision is that for some reason and nobody knows why, this area heals with a scar nearly invisible in most patients.
Breast augmentation is performed outpatient. In this practice it is performed under general anesthesia. Although some practices perform breast augmentation with sedation and local anesthesia, I think it's a much more pleasant experience and this can be done more precisely with what you sleep. Risks general anesthesia are usually greatly exaggerated.
In fact, most patients have a much higher risk injuries and deaths into their car by the sophisticated techniques now available general anesthesia. I do not use drains, or tubes to collect fluid that may occur around the implant. I know that some surgeons do, but I found no advantage to use. However, I would not be the fault of a surgeon and I use the occasion to use them well. If used, they are usually removed 24 hours later.
We always see you the day after surgery. We'll give you a bra postoperatively at that time. We'll see you again in 7 to ten days for suture removal and again at 6 weeks after surgery to ensure that implants are well settled in their pockets and everything runs normally. I would like to see every year to check and see how you do.
Most patients take about 7 days off work. I have only two major rules after breast augmentation. No bra or push-up sub-wire until the implants are installed (about 6 weeks) and if it hurts, do not do it. Most patients can lift 10 to 15 pounds for the first two weeks. Mild exercise can usually be launched in two weeks, but the work outs upper body should be laid off for four to six weeks.
It takes about six weeks to see the final result with any type of surgery. However, areas of the body that have undergone surgery actually change and mature for a full year. The change from six weeks to one year are very subtle and occur slowly.
It can be wise to wait before having children before having breast augmentation. Due to the risk of infection, you should not breastfeed in 8 months to have surgery. Furthermore, to become pregnant immediately after surgery can compromise the result because of the hormonal stimulation large chest of pregnancy. There are two other concepts that are very important for you to understand. First is the concept of cup size. Cup size is the worst way to measure the chest of a individual for many reasons. First, few people understand what is really meant by cup size. Few women wear the right size bra and all manufacturers of bra do differently. As implants become larger, their diameter at the base becomes larger and they can sit too close to each other. This seems ridiculous. The implants also come in many sizes. Because of these factors, there is no guarantee for a cup size after surgery on a patient particular. However, I always ask what the patient wants to know what they think.
Patients who come to Desiring to be a C, usually really want a D, but are afraid to say that the thought that D is too large. What this means to me is they want to be proportional to their size and look good in their clothes and swimsuits. They do not want to be jokes or their relatives in the fighting obtain on them. The women who come in saying they want to be a D generally want to be much larger.
Usually as big as I can possibly go. Sometimes patients present with pictures from magazines and say "I want to see him like that." The reality is that we do not usually have much flexibility as breast augmentation and your individual anatomy will dictate your final result. Another concept you must be comfortable, is that breast augmentation is by its nature an operation maintenance.
